I hate it when th field of obstetrics is prioritised over other fields. No one chooses to get ill or injured but no one is forced to have kids, it's a choice.

Many women would rather not have children, but see it as an obligatory aspect of marriage, so much so in many cultures a wife can be "traded in for defects" for a "new defect free model" if for some reason, many times nothing to do with her, she has not gotten pregnant.

Women are in a vulnerable state when they are pregnant. Especially with high-risk pregnancies, which are numerous different reasons. They need a specialist. A small town, do-it-all, country doctor who has been practicing since 1960 isn't going to cut it anymore. The guy still thinks that a penicillin shot in the ass cures all infections, that birth control should only be for married couples over 25 who already have two kids, wounds should be cleaned thoroughly with rubbing alcohol, the polio vaccine should be administered on a sugar cube to a bunch of kids lined up at school with their tongues sticking out, and "dia-beet-us" can be treated with a shot of bovine or pork insulin.
